Which temperature column should I use?

Use the column allowed by the termination rating and the conductor insulation. When they differ, the lower one usually governs.

Why does the wire size calculator sometimes pick a larger size than this chart?

Voltage drop. Ampacity can pass while the long-run percent fails.
